GENERAL INFORMATION FOR

2008

CONNECTICUT BREAKTHROUGH QUALITY AWARD

AND

CONNECTICUT LEADERSHIP QUALITY AWARD

 

1) Criteria – 2008 Baldrige National Quality Program Criteria for Performance Excellence (referred to as the Business/Nonprofit Criteria), 2008 Education Criteria for Performance Excellence, or 2008 Health Care Criteria for Performance Excellence. The Criteria booklets a may be downloaded from the Baldrige NIST website at www.baldrige.nist.gov/Criteria.htm.

2) Application Deadline – Thursday, May 22, 2008. Ten copies

3) Site Visit – Regardless of numerical score on the written application, all 2008 Connecticut Breakthrough Quality Award and 2008 Connecticut Leadership Quality Award applications will receive a full-day site visit from no less than four, possibly five, 2008 Baldrige trained Connecticut Quality Improvement Award examiners at the beginning of July 2008.  A score of no less than 525 on the written application must be reached to receive the 2008 Connecticut Breakthrough Quality Award or the 2008 Connecticut Leadership Quality Award.  The purpose of the site visit is to encourage all associates to be part of the clarification and verification process used by their organization and CQIA examiners.

4) Examiners – At least one active 2008 Malcolm Baldrige National Quality Award examiner from Connecticut plus four 2008 Baldrige trained Connecticut Quality Improvement Award examiners will read each application and actively participate in the writing, consensus and all-day site visit for each application. After the site visit, CQIA examiners will prepare a shorter feedback report on the site visit in addition to the written feedback developed at the earlier consensus meeting held at the beginning of July 2008.

5) Feedback – All written feedback will be completed and returned to the applicant by the last day of August 2008.

6) Applicant’s CQIA Examiner – Each applicant for the Connecticut Breakthrough Quality Award and Connecticut Leadership Quality Award will have one knowledgeable, conscientious and dependable associate serve for no less than two years on the CQIA Board of Examiners and attend the 2008 CQIA Baldrige training session. If for any reason that organization’s examiner cannot serve for the entire 2008 season, a replacement must be sent immediately or the organization’s application will be return at that time without feedback or refund of fee.

7) Possible Conflict of Interest – CQIA’s 2008 examiner team will not include an examiner who may have a conflict of interest with the applicant organization.  All examiners will be thoroughly questioned.

8) Both CQIA examiners and applicant organizations will be treated at all times with courtesy and professionalism.

9) Intent to Apply – All interested applicant organizations must submit An Intent to Apply  to Sheila Carmine, Executive Director, CQIA, PO Box 1396, Stamford, CT 06904-1396 no later than March 1, 2008. The Intent to Apply will contain the entire non-refundable application fee of $4,500 which includes written CQIA examiner feedback for the organization’s written application as well as shorter CQIA examiner feedback for the all-day site visit. Intent to Apply fee will be mailed must be received at CQIA, PO Box 1396, Stamford, CT 06904-1396 no later than March.
